Personal History Statement (PHS)

The Sheriff’s Department uses a Personal History Statement (PHS) to learn more about its candidates for all Deputy Sheriff classifications.

If you pass the PIQ, you will be given instructions by a background investigator on how to fill out the PHS online, which you can also download below. Please DO NOT BRING your pre-filled PHS to the pre-screen orientation, it will not be accepted!

Your assigned background investigator will then be your main point-of-contact throughout the remainder of the hiring process. He or She will schedule the remainder of interviews/appointments for you.

How you complete the PHS packet can determine how successful you are at moving on to the next step in the process. Please read and follow the directions provided to you and pay attention to detail. Your background investigator will also provide a link to a video with instructions.
